Telehealth Consent
Telehealth involves the delivery of health care services using electronic communications between a licensed physician and a patient not in the same physical location. Prevsi Health uses a secure, HIPAA-compliant platform governed by Florida Statute §456.47.

Risks
As with any medical care, there are potential risks, including interruptions of service, unauthorized access despite safeguards, and limitations inherent to evaluation without a physical exam. In rare cases a physician may determine that an in-person visit is required.
Your rights
You may withhold or withdraw consent at any time without affecting your right to future care. You have the right to request a copy of your medical records, which will be provided at reasonable cost and in accordance with Florida law.
Prescriptions
Any prescription issued will be based on the physician's clinical judgment following review of your health questionnaire and laboratory results.
