Before you start
Please read this consent carefully. It explains how telehealth care through Prevsi works so you can decide whether it is right for you. If you have questions, contact our support team Monday to Saturday, 9 a.m. to 9 p.m. Eastern Time, before you agree.
What telehealth means
Telehealth is health care provided at a distance using technology. Through Prevsi, a licensed Florida provider reviews the information you share online, orders testing and may prescribe medication. Depending on your needs and Florida law, your provider may also ask for a phone or video visit.
Prevsi is not a pharmacy or a laboratory. Prescriptions are filled by licensed pharmacies, and tests are processed by certified laboratories.
Not for emergencies
I understand that I should never use Prevsi for a medical or mental health emergency. In an emergency, I will call 911 or go to the nearest emergency room. If I may have been exposed to HIV in the last 72 hours, I will seek care right away and ask about PEP. For a mental health crisis, I can call or text 988.
When my care begins
I understand that a provider becomes responsible for my care only after I complete the required intake and the provider reviews my information and accepts me as a patient. Creating an account, answering questions or sending a message does not by itself create a provider–patient relationship.
My provider may decide that telehealth is not right for me and refer me to in-person care. Messages may not be reviewed until the next business day.
Services covered
I understand that Prevsi offers only PrEP, at-home STI testing and long-term HIV care, and that I need to get other medical care elsewhere. General content on the site is for education only. Medical advice comes from my provider.
Benefits
- Care from home, on my own schedule, without travel or waiting rooms.
- Discreet testing and medication delivery.
- Easier follow-up and ongoing care through my account.
Risks and limits
- My provider cannot examine me in person, so some conditions could be missed that an in-person visit might find.
- Care depends on the information I give. If it is incomplete or inaccurate, my care could be affected.
- Technology problems could delay my care.
- Despite security safeguards, electronic information could be accessed without permission.
- Any medication can cause side effects or interact with other medications.
Testing
- No test is 100% accurate. Results can be falsely positive or negative.
- Testing too soon after an exposure can miss an infection. My care team may recommend testing again later.
- A positive result may need a confirmation test, and some infections need an in-person evaluation.
- Florida law requires certain infections, including HIV, syphilis, gonorrhea, chlamydia and hepatitis, to be reported to the Florida Department of Health.
- Samples must be collected and returned as the kit instructions describe.
My responsibilities
- Give complete and honest answers, and tell my care team about changes in my health or medications.
- Read the information and messages my care team sends, and follow up as asked.
- Use only one account, and use the service only for myself.
- Seek in-person care when my provider recommends it or my symptoms get worse.
Medication history and pharmacies
I agree that my provider may review my prescription history from pharmacies and prescription networks so they can prescribe safely. I authorize Prevsi to arrange for a licensed pharmacy to fill and deliver prescriptions my provider sends.
Privacy
My information is protected as described in the Privacy policy. Telehealth records become part of my medical record, and I have the same rights to them as to in-person records.
My choices
- I can choose not to use telehealth and seek in-person care instead.
- I can withdraw this consent at any time by contacting Prevsi. Withdrawing does not affect care already provided.
- I can ask questions about this consent at any time.
My agreement
By checking the consent box during enrollment, I confirm that I am at least 18 years old, I live in Florida and will be in Florida when I receive care, I have read and understand this consent, and I agree to receive care through telehealth.
